Transcripts

Transcripts show a complete course history, including all grades and withdrawals. UCSB Extension does not send transcripts to UCSB’s Registrar or Office of Admissions automatically. Transcripts must be formally requested in writing by the student. Transcript fees are listed online at www.extension.ucsb.edu/student/transcripts and in the Fee Chart

To receive a transcript, you may use the Transcript Order Form available online at www.extension.ucsb.edu/student/forms, or send your written request (by mail or online) to the Student Services office; all of the following information must be included:

  • Full name
  • Social Security Number
  • Your address and telephone
  • Address(es) where transcripts are to be sent
  • Number of copies
  • Include payment
  • Dates you attended UCSB Extension
  • Note if transcripts should be held for current quarter grades
  • Signature (required to process)
